Provider types in the directory are boarding care homes, home health agencies, home care providers, hospices, hospitals, housing with services, nursing homes and supervised living facilities and other non-long term care providers. Providers can be identified by type, county, city or name. This page provides a link to download current data from the MDH database. The link works best in Internet Explorer and Firefox. This data is provided in tabular format. There is no assoicated geographic dataset; results require geocoding to be mapped. A link to the file with the field names and definitions is also provided below.
